Job SummaryProvide case management services to Mental Health Court and Drug Court participants, Conduct mental health, substance abuse and risk evaluations, Develop person-centered treatment plans, Facilitate groups, Create staffing summaries for client staffing, Perform and observe Drug testing for participants, Provide referrals for substance abuse treatment, employment, education, transportation, mental health services, other social services and entitlements, Participate in staffing and court calls for Mental Health Court and Drug Court, Conduct site visits for treatment agencies, Collect and enter data entry for MIS system and performance measurement reports, Maintain client folder of information, Maintain working relationships with treatment facilities, the probation department, the judge, the Health Department, and other social service agencies, Attend conferences and workshops (ILAPSC, All Rise), Occasional client transport, Perform any and all duties as assigned.
Minimum QualificationsMaster’s Degree in social work, psychology or related field, License and CADC preferred, Ability to interview individuals, diagnose and write mental health assessments and person-centered treatment plans. Familiarity with Microsoft Office, Good oral and written communication skills, must undergo background check and driving abstract, detail oriented and solid organizational skills, dependable, consistently on time and prepared for work, follow instructions, respond to management direction, solid interpersonal skills, must maintain confidentiality, must be able to prioritize, plan and use time efficiently.
Salary Range$50,000 - $55,000/yr.
